Statement regarding Patient Rights in the Clinic

We hereby declare that in relation to the Criteria of participation in the “Friendly Clinic – Networking” Programme – the Introduced Standards of Patient Service:
1. The following messages are available and visible to Patients in the Clinic’s waiting hall:
- Information about the Patient’s Rights as provided for in the Law of November 6, 2008 on the patient’s rights and the Commissioner for Patients’ Rights (Dz.U. [journal of laws] of February 13, 2012, Pos. 159, as amended),
- Information about the address and telephone number of the Commissioner for Patients’ Rights,
- Information about the procedure of filing complaints and requests in the facility, that is the Clinic,
- Information about addresses and telephone numbers of entities providing 24-hour-a-day medical care within the scope of Primary Health Care, in case the Facility realises procedures within the scope of POZ,
- Information about medical emergency numbers (112, 999), and
2. All medical cabinets of the Facility guarantee the Patient intimacy during medical examination or medical procedures requiring undressing.
